What a Reference Range Actually Is (and Isn't)
This is the single most misunderstood part of a lab report, so it's worth being precise.
A reference range is not a definition of health. It's a statistical interval, usually built so that roughly the middle 95% of a defined reference population falls inside it. Two consequences follow directly from that, and both matter:
- Being outside the range doesn't automatically mean something is wrong. By construction, about 1 in 20 healthy people fall outside the range on any given test. Run a panel of 30 markers and it's statistically ordinary to see one or two flags on a perfectly healthy person.
- Being inside the range doesn't automatically mean everything is fine. A value can sit at the very edge of "normal," or drift substantially within the range over years, and still be worth a physician's attention in context.
Ranges also vary by lab, because they depend on the instruments, methods, and reference population each lab uses. That's why comparing a result from one lab against another lab's range can be misleading, and why the range printed on *your* report is the one that applies to *your* result. What the Common Panels Measure
Most routine bloodwork is some combination of four or five panels. Here's what each one is actually looking at — mechanistically, not diagnostically.
Complete Blood Count (CBC)
Counts and characterizes the cells in your blood. Red blood cells, hemoglobin, and hematocrit relate to oxygen-carrying capacity. White blood cells are immune cells, often reported with a differential breaking them into subtypes. Platelets are involved in clotting. Indices like MCV describe the average size of red cells.
Comprehensive Metabolic Panel (CMP)
A fourteen-marker panel covering several systems at once. It includes glucose; electrolytes such as sodium, potassium, chloride, and CO2; kidney markers including BUN and creatinine; liver markers including ALT, AST, ALP, bilirubin; and proteins such as albumin and total protein. A Basic Metabolic Panel (BMP) is the shorter version without the liver markers.
Lipid Panel
Measures circulating fats: total cholesterol, LDL, HDL, and triglycerides. Some panels add ApoB or Lp(a), which describe lipid particles in more detail than the standard four.
Thyroid Panel
TSH is the pituitary signal telling the thyroid to produce hormone, which is why it's usually the first-line test. Free T4 and Free T3 are the circulating thyroid hormones themselves. Because TSH is a signal rather than the hormone, it moves inversely to thyroid output, which surprises a lot of people reading their own report.
Hemoglobin A1c
Reflects average blood glucose over roughly the preceding three months, by measuring the fraction of hemoglobin that has glucose attached. It's a longer-run view than a single fasting glucose, which captures only that moment.
Hormone Panels
Depending on what's ordered, these may include testosterone (total and free), estradiol, progesterone, LH and FSH, SHBG, prolactin, or cortisol. Hormones are especially context-dependent: many follow a daily rhythm or a cycle, so *when* the blood was drawn can matter as much as the number itself. This is a category where interpreting a single value in isolation is particularly unreliable.

Five Things That Change What a Result Means
The same number can mean different things depending on conditions at the draw. Common factors:
- Fasting status. Glucose, triglycerides, and some other markers shift meaningfully after eating.
- Time of day. Cortisol and testosterone both follow a daily rhythm and are typically drawn in the morning for that reason.
- Recent exercise. Hard training before a draw can transiently move muscle-related markers such as CK, and can affect others.
- Hydration. Being dehydrated concentrates the blood and can nudge several values upward.
- Medications and supplements. Plenty of both can influence lab values, including biotin, which can interfere with certain immunoassays such as some thyroid tests.
None of this is a reason to distrust your labs. It's a reason your physician asks about the circumstances of the draw before drawing conclusions.

Why One Flagged Value Rarely Tells the Story
Three principles physicians apply that consumers reading their own reports often miss:
Patterns beat singles. Related markers move together. A cluster of related values shifting in the same direction carries far more information than one isolated flag.
Trends beat snapshots. A value that has moved steadily across three annual tests is more informative than any single measurement, even if every one of those values sat inside the range. This is the strongest argument for testing consistently over time rather than once.
Context beats numbers. Your symptoms, history, medications, age, and sex determine what a value means. The same result can be unremarkable in one person and worth investigating in another. A number without a person attached to it isn't interpretable, which is the fundamental limit of reading your own labs.

What to Do With Results You Don't Understand
- Don't panic at a flag. As above, isolated out-of-range values are statistically common and frequently benign.
- Write down your questions before your appointment: which markers were flagged, what changed since last time, and what you want to understand.
- Ask about the trend, not just the value: "How does this compare to my last test?" is often the more useful question.
- Ask what, if anything, changes as a result. Sometimes the answer is "we recheck in six months," and that's a legitimate answer.
- Bring context. Fasting status, timing, medications, supplements, recent illness or training.
- Be careful with internet symptom-matching. Searching a flagged marker reliably surfaces the most alarming possible cause, which is almost never the most likely one.
